Our initial service offering is targeted at the burgeoning problem of diabetes in the United States, and all its co-morbidities. Diabetes is a disease that requires patient compliance with complicated and long term medication regimens for successful management. Many patients cannot sustain this regimen without intensive personal case management by a qualified practitioner.

Traditional case management relies upon either home visits or phone calls. By contrast, Informed Care utilizes equipment and protocols that combine video conferencing with nurse oversight and care coordination with the patients' physician.
